Data Collection Requirements

The 2015 calendar year is the first year that Utah's youth courts were required to gather this data. The Utah Youth Court Association is in has developed a statewide Youth Court Database that all youth courts will use to input and record data for all referred youth offenders and youth volunteers.

Youth courts are encouraged to use the "UYCA Data Collection Survey," to prompt families for most of the data we are required to collect. The referred youth survey, which can be downloaded below, does not have identifying information on it. Instead, each youth court will assign referred youth a unique case number. The case number should be filled in on the upper right hand corner of the survey before the survey is given to the referred youth's family. Each youth court can then input the data from the "UYCA Data Collection Survey" into the Airtable form located here.

Do not share identifying information on referred youth offenders or youth volunteers with anyone who has not been given legal authority to view that information. Once the data reporting cycle begins, the Utah Youth Court Association will send out specific instructions for submitting non-identifying data to the Utah Youth Court State Advisory Board.
